This request is required if you will be enrolling in two colleges during the same term. It is necessary for the tracking of your satisfactory academic progress and the transferability of these courses back to MSU.

Click the submit button to send your request. Once this form is processed through our office, you will receive a confirmation letter through the mail. If the school you are attending is a quarter hour school, it is your responsibility to indicate that when requesting concurrent enrollment.
