BSRS Students:

In recent years we have added clinical components to some of our bachelor level courses. We will be adding more clinical components to the bachelor level courses in the future. Therefore, it is mandated that we keep immunization records on all of our students that will be enrolled in these courses.

We require a copy of your official immunization records. In addition, you must complete a Medical History form. If you are current with the immunizations listed, just send us a copy of that record. If you are not current on these immunizations, you must become current as dictated by law. Please note that self-declared immunizations or immunizations submitted on a questionnaire or a medical physical form cannot be utilized.

When you are on campus, you may go to the Vinson Health Center for these immunizations at a reduced cost. You may also obtain immunizations at your local public health department at a reduced cost.

Completion of all immunizations is required before you can register. (See registration information.)

Please forward the information to the Department of Radiologic Sciences at Midwestern State University. If you have any questions, please call the BSRS program secretary at 1-866-575-4305.
