Thank you for your interest in our program! The application consists of four separate forms listed below. In addition, we request that you provide a signed copy of your primary parent/legal guardian’s 2025 IRS 1040 Income Tax Forms. We can accept forms in person or via email at SSS@msutexas.edu. If you did not file, plan to file, or have any questions, please contact Student Support Services at (940) 397-4135 or via email.

Upon receipt of all required items, the Program Director will review your application, and you will be notified of a decision via email and postal mail.
