IMPORTANT 2025 TAX FORM 1095-C NOW AVAILABLE

Per updates to the Affordable Care Act (ACA),  employers are no longer required to automatically mail employees their 1095-C tax form. The 1095-C tax form indicates an offer of insurance coverage for the reporting year.

 You may access your 1095-C form electronically on Banner 9 Self-Service by following the steps below.

 Provide electronic consent for 1095-C tax form:

  • Log into Banner 9 Self-Service
  • From "Employee Dashboard"
  • Select “Taxes”
  • Select “Electronic Regulatory Consent”
  • Click on the box next to “Consent to receive 1095-C electronically” to populate a check mark
  • Select “Submit”

 To view / print your 1095-C tax form:

  • Log into Banner 9 Self–Service
  • From "Employee Dashboard"
  • Select “Taxes”
  • Select “1095-C Employer Provided Health Insurance Offer and Coverage Statement”
  • In the “Tax Year” drop down menu, select the correct year to view your 1095-C, click “Display”
  • To print the 095-C, click “Printable 1095-C" at the bottom right corner of the screen

If you choose not to provide electronic consent, you may request your printed 1095-C form by emailing liza.villadiego@msutexas.edu.  Your 1095-C form will be mailed to your address on file within 30 days from the date of your request or you may pick it up in the Human Resources office located in room 210 of the Hardin Administration Building.
